Do you have a passion for Asthma / COPD or Diabetes. We have an exciting opportunity at Lakeside Healthcare St Neots for a registered nurse who wants to make a difference in our patient's lives. We are a large friendly practice with a great team of existing Nurses / AHPs.,

  • Responsible for providing and delivering primaryhealth care interventions and care effectively and efficiently
  • Working collaboratively with themulti-disciplinary general practice team and other healthcare professionals topromote integrated pathways of care to meet patient needs.
  • Effective communication with patients, carersand all members of the MDT
  • Effectiveprioritisation and completion of daily work in line with clinical patientneed and practice priorities
  • Maintenance of accurate and legiblepatient records in line with professional and organisational requirements this includes contemporary paper records and electronic patient recordsystem
  • Actively participates and contributes to nurseteam meetings
  • Initiates health promotion activities withindividuals, patients and carers
  • Participates in clinical audit and theimplementation of changes to ways of working and / or clinical practice as aresult of findings, recommendations and action plans
  • Proactive in improving and influencing thepatient care pathway / journey
  • Participates in risk assessment and promotes thesafety and well-being of patients, staff and any other persons at all times andassist in ensuring a safe work environment
  • Ensure that any complaints or incidents areidentified and reported to the lead GP, Nurse Manager or Practice Manager inaccordance with the organisational policy and expected professional practice, Thepurpose of the post is to deliver safe, high quality primary care nursingservices to the practice population. Working within the NMC Code of Conduct andprofessional boundaries, the post holder will deliver primary health careinterventions and care. The post holder will support service development, thesafe delivery of policy and procedures, and demonstrate effective professionalnurse standards, advocacy and performance., Responsible for providing and delivering primaryhealth care interventions and care effectively and efficiently

  • Monitors and orders stock levels and equipmentin line with budget and lean principles
  • Responsible for the security, care andmaintenance of equipment ensuring standards of infection control and safety aremaintained
  • Ensureclinical practice is safe and effective and remains within boundaries ofcompetence and to acknowledge limitations.
  • Duties include-
  • Providing treatment services and healtheducation advice : including Dietary advice, blood pressure checks, urinalysis,health screening, Ear syringing, dressings, ECG, Phlebotomy, routineinjections, sexual health, contraception, routine immunisations andvaccinations, removal of sutures / clips, baby immunisations, assisting withthe fitting of IUCD / IUS, first registration checks / health surveillance / general health advice, assisting with minor procedures in the treatment roomand acting as chaperone as required
  • Provide general and specific health screeningsto the practice patients (within agreed protocols) with referral to othermembers of the primary and secondary health care team as necessary
  • Working within guidelines and protocolsundertake and record :
  • Urinalysisand preparation of specimens for laboratory analysis
  • Temperature,pulse, blood pressure and respiratory rate
  • Weight,height, waist circumference and Body Mass Index
  • Measurementof legs for compression hosiery
  • ECGs,ambulatory blood pressure monitoring
  • Nearpatient testing blood glucose, lipids and INR
  • Dopplerultrasound measurement working with registered nurse
  • Measurementof airway function (peak expiratory flow rate and spirometry)
  • Obtain venous blood samples from individuals forinvestigation.
  • Administer medications by injection with theappropriate legal authorisation.
  • Recognise health promotion opportunities andgive brief, focussed lifestyle advice using motivational interviewingtechniques which may include :
  • Alcoholscreening
  • Smokingcessation
  • Weightmanagement
  • Sexualhealth
  • Undertake initial assessment of patientspresenting with wounds, working within expertise to manage independently orrefer to other services as necessary.
  • Remove wound closure materials includingsutures, clips and staples.
  • Take samples for the cervical screeningprogramme according to recognised standards.
  • Provide ear care including removal of cerumenand provide education and advice.
  • Administer vaccines to adults and childrenaccording to the UK immunisation schedule.
  • Provide a pre-travel health service includingadministration of vaccines and advice after undertaking appropriate riskassessment.
  • Administer prescribed therapies such ascontraceptive injections, vitamin B12, and depo neuroleptic medicationsrecognising side effects and contraindications.
  • Develop and follow referral pathways forpatients with specialist needs.
  • Minor Illness
  • Support the Same Day / Urgent Care services asdefined by role and competence
  • Triage and treat patients wishing to see ahealth care professional making any necessary referrals to other members of theprimary health care team.
  • Ensure clinical practice is safe and effectiveand remains within boundaries of competence and to acknowledge limitations.
  • Advisepatients on general health care and minor ailments with referral to othermembers of the primary and secondary health care team as necessary
  • Long term conditions : -
  • Assist nurse-run long term condition clinics andcarry out clinical measurements and near patient testing for patients withdiabetes, asthma and COPD in line with NICE guidance and Quality and OutcomeFramework indicators which may include : bloodsugar monitoring assessmentof feet with appropriate risk stratification spirometry pulseoximetry
  • PeakExpiratory Flow rate teaching inhalertechnique assessment of breathlessness.
  • Recognise signs and symptoms that may indicateundiagnosed long term conditions poor wound healing, winter cough insmokers and also factors that may indicate mental health problems and userelevant referral pathways
  • Workin accordance with Practice protocols to ensure the safe and effectivemanagement of patients with Chronic Diseases.
  • Pathological specimens and investigatory procedures :
  • Undertakethe collection of pathological specimens including intravenous blood samples,swabs etc. Perform investigatory procedures requested by the GPs
  • Other administration and professional responsibilities :
  • Record accurate consultation data in patientsrecords in accordance with the latest NMC guidance and other pertinentstandards
  • Maintain professional registration and practicein accordance with NMC requirements

  • Participate and support service improvementinitiatives

  • Using professional skill and expertise, activelyparticipate in the education and training of students of all disciplines andthe introduction of all members of the practice staff where appropriate

  • Willingness to undertake training and education to achieve core skills and experience required of a Practice Nurse
