Practice Nurse (Full-Time or Part-Time)

We are seeking a dedicated Practice Nurse to join our friendly and supportive team to , Deliver high-quality nursing care, including chronic disease management, immunizations, wound care, and health promotion. Collaborate closely with GPs, fellow nurses, and other healthcare professionals to provide comprehensive patient care. Conduct thorough patient assessments, develop and implement care plans, and regularly evaluate patient outcomes. Provide support to patients in managing their health conditions, offering guidance on leading healthier lifestyles. Maintain accurate and detailed patient records while ensuring confidentiality is strictly upheld. Actively participate in team meetings and contribute to the continuous improvement of our services.
