We have an excellent opportunity for a Practice Nurse to join a growing establishment committed to being a flagship organisation and an at scale provider of primary care services in the Midlands. If you are enthusiastic, caring and passionate about delivering the highest quality nursing care, keeping patients at the heart of all you do, we want to hear from you., You will work collaboratively as part of our multidisciplinary sector team of clinicians to meet the needs of our patients; delivering the highest standards of patient care. You will undertake a diverse range of nursing assessments and provide the appropriate care / treatment according to patient needs, striving to improve standards of care and enhancing nursing services within the practice.
Depending on experience, Long Term Chronic Disease management will also form a large part of the role as well as the mentoring of our Health Care Assistants, Assistant Practitioners, Nurse Associate Students and RGN Student Nurses. Knowledge of the GMS contract e.g. The Quality and Outcomes Framework, will be valuable to the post., If you're looking for flexible or predictable working arrangements, please speak to us about how we might be able to accommodate this. This role is part of the integrated health and social care community across Derbyshire, where there are many opportunities to work across different locations and providers developing new and different skills. Whilst this role is initially based in Chesterfield there may be future opportunities to be based at different locations within the health and social care community which we may talk to you about in the future.
