Practice Nurse with Diabetes Specialism

We are looking for an enthusiastic Experienced Practice Nurse who has a Diabetes Specialism to join our expanding team of nurses at our friendly practice. Lister House Surgery consists of 4 sites across Derby and you may be required to work at any of these sites depending on the business needs.,

• To have a particular interest in diabetes
• To implement and evaluate individual treatment plans for patients with diabetes, that promote health and wellbeing
• To work as part of the Practice Nurse team, providing general practice nursing services to patients
• To assist with the leadership and good management within the nursing team
• Work autonomously following local and national guidance
  
  We are GP teaching practice with a dedicated team committed providing specialist management of long-term conditions and clinics covering a wide range of healthcare issues for Derby City and the surrounding area.
