Full Job Description

1. To provide clinical leadership, in relation to medicines management, and, as a senior clinician, contribute to the development of primary care services for the population served. 2. To provide specialist pharmaceutical advice, on a day-to-day basis, in all aspects of prescribing and medicines management to GPs, nurses, other practice staff and patients as appropriate. 3. To monitor practice prescribing costs, and local and national prescribing targets, reviewing prescribing patterns, financial changes, and adherence to local guidelines and formularies. 4. To develop practice specific protocols and formularies in line with agreed CCG and National guidance, to support high quality, safe, cost-effective, and evidence-based prescribing. 5. Oversee the repeat prescribing reauthorisation process ensuring that it is safe and robust for all patients. Ensure that there is a safe review process for patients on high risk drugs and for patients at risk of admission. 6. To undertake clinical
medication review and support the management of long-term conditions and self-care within domiciliary or community settings, as appropriate. Reducing inappropriate polypharmacy and wasteful prescribing. 7. To support the implementation, of CCG initiatives around long-term conditions and self-care, providing health promotion and preventative healthcare advice when necessary and liaising with other healthcare professionals as appropriate e.g. secondary care providers, chronic disease management team, and integrated care team. 8. Ensure that medicines are reconciled in a timely manner and work with patients, GPs, carers and care providers and other health professionals to ensure patients receive the medicines they need in a timely manner. 9. To provide phone advice regarding medicine related queries and minor ailments and be able to triage appropriately. 10. Working within professional boundaries and competencies, provide face to face clinical advice in relation to medicine related queries
and minor ailments. 11. To maintain clear, accurate and contemporaneous computerised records of all patient contacts. 12. To work with prescribers and GP trainees to undertake clinical audit as appropriate, within the practice to demonstrate NICE implementation, as part of CCG work plan or as a result of identified clinical issues within the practice. 13. To assist in the education and training of healthcare professionals employed within the practice, in relation to medicines and the prescribing system. 14. To assist the practice in managing the introduction of new drugs, and drugs that have been withdrawn. 15. Co-ordinate the response to medicine related alerts received from the MHRA and Drug companies according to agreed local policy and guidance. 16. To work with local pharmacies to address drug supply shortages causing minimum disruption to patient care. 17.
