An exciting new opportunity for a Practice Pharmacist to join our dynamic friendly practice team. If you enjoy face to face contact with patients and treating minor ailments then please apply. You will also be responsible for leading our well established pharmacy technician team. In collaboration with other clinical and managerial colleagues, you will maintain and improve quality general medical services and the management of prescribed medication for the registered patients of the Practice. You will be able to actively participate in specific clinical areas of interest eg pain management and mental health. You will also work towards the achievement of the clinical elements of the GMS contract and prescribing indicators. Main duties of the job As a Practice Pharmacist / Independent Prescriber you will deliver a daily Minor Ailments service. You will be supported by the clinical team. There will be opportunities to develop the role further. You will work within your scope of practice with the freedom to autonomously manage patient care. You will identify, assess and diagnose patients with complex health needs across the practice. You will undertake patient medication reviews within the surgery, prescribe and update medications You will lead the prescribing team and you will be responsible for supporting the leadership and ongoing training and mentoring of the team. You will have ongoing opportunities for continued professional development, mentorship and support, to ensure that you are competent and confident in your role. In addition you will be directly responsible for leading several clinical areas. About us We are a dynamic friendly coastal medical practice with surgeries in Llantwit Major, St Athan and Rhoose, providing high quality general medical services to 10500 patients. We currently have 5 GP Partners, 3 Salaried GPs, 1 Nurse Practitioner, 3 Practice Nurses, 2 Healthcare Assistants, 2 Pharmacy Technicians, a Cluster Mental Health Practitioner and a Cluster Physiotherapist. We employ around 20 members of staff across Management, Administrative and Reception Areas. We offer an extensive range of enhanced services. We are an experienced training practice and we have been training General Practitioners for over 20 years. We are innovative and constantly developing new systems to improve internal processes. We are proud of our practice team, and we prioritise a team approach by holding daily virtual meetings via teams for all clinicians to provide support, feedback and guidance.,
- To provide a comprehensive range of clinical duties including :
- Identify, assess and diagnose patients with complex health needs across the practice population working to protocol and in close liaison with doctor
- Action acute prescription requests from patients and prescribe where appropriate or arrange blood tests. Refer to GP if necessary in line with practice protocols.
- Undertake telephone consultations on medication issues and update medication where appropriate
- Reauthorise and prescribe repeat chronic disease medications following patient reviews.
- Action practice GMS Contract prescribing issues and prescribing indicators polypharmacy reviews and report writing for submission.
- Provide training to registrars / GP trainees / medical students and induct new Clinicians in practice on medicines management strategies and prescribing targets
- Provide prescribing support for all Clinicians as required.
- Aid compliance with practice and UHB formularies.
- Review, check and update medication from hospital discharge summaries for all medications including insulin and epileptic medications.
- Manage the shared care medications process within the practice, to include interpretation of blood results and prescribing. To contact patients where needed to explain blood results that are out of range and to liaise with secondary care for further advice when needed.
- Write referrals to secondary care (as appropriate) under the supervision of clinician
- Undertake incentive scheme audits feedback to prescribers, monitor improvements and ensure improvement is maintained.
- Maintain / update medicines management protocols in line with latest guidance chronic disease management
- Undertake patient medication reviews within the surgery, prescribe and update medications
- Complete End to End review of the current repeat prescription process, including bulk prescribing, in line with the continuous improvement program within the practice.
- Complete End to End review of the current acute prescription process in line with the continuous improvement program within the practice.
- To attend and contribute to regular practice clinical and administrative meetings whether formal or informal. This may involve adjustments to workload to allow attendance.
- To support the practice in the implementation and achievement of the GMS contract. To take an active role take in providing practice enhanced services.
- To contribute to the clinical governance of the practice, and to fully participate in the reporting of incidents to the clinical governance lead.
- To keep up to date with current and forthcoming National Service Frameworks and NICE guidance and their impact on primary care services.
- To ensure that care is delivered using evidence based practice by developing and maintaining your level of expertise, knowledge and skills
- Ensure that adequate, accurate and appropriate clinical records are maintained for all work carried out in the practice.
- To work as part of the clinical team and to organise and prioritise workload on a day to day basis. This will include :
