Full Job Description

The role involves working within the NationalFramework for NHS Continuing Healthcare and NHS-funded Nursing Care todetermine eligibility for Fast Track continuing healthcare. You will beresponsible for the review of completed Fast Track applications, liaison withreferring professionals and close partnership working with system wideservices. You will also liaise with hospital trusts in respect of applicationsfor patients awaiting discharge. You will participate in the commissioning andreviewing of packages of care to ensure they are safe and sustainable insupporting an individuals assessed needs. You will also be responsible forreviewing eligibility should someones needs change. The role also providesopportunity to assist in the delivery of Fast Track training to colleaguesacross the system to aid understanding of the National Framework and supporthigh quality and timely applications., The Health and Care Bill puts ICSs on a statutory footing empowering them to better join up health and care services, improve population health, and reduce health inequalities. ICSs have four core purposes :

  • Improve outcomes in population health and healthcare
  • Tackle inequalities in outcomes, experience, and access
  • Enhance productivity and value for money
  • Help the NHS support broader social and economic development, The post holder will play a keyrole in supporting the Personal Health Commissioning Service. Working withinthe remit of the National Frameworks for Continuing Healthcare, the post holderwill deliver an effective and competent level of service and consistentlydeliver a client focused service.
  • The post holder will beresponsible for assessing and reviewing patients, ensuring The NationalFramework for NHS Continuing Healthcare and Funded-Nursing Care is implementedand complied with, according to the principles and processes. Ensuring valuefor money that meets the needs of the individual and delivery of best practicehealthcare.

    TheFast Track team are seeking an experienced registered nurse or allied healthprofessional motivated by providing outstanding person-centred end of lifesupport., The successful candidate must be able to workcollaboratively with Health and Social Care practitioners, patients, familiesand carers, ensuring a responsive and high-quality service. You will be empathetic, honest and haveadvanced communication skills with the capacity to hold compassionate ordifficult conversations. You will have a flexible approach and be able to workautonomously. A knowledge of the National Framework and / or experience ofworking in end-of-life services is desirable. You will have a sound understanding ofsafeguarding principles and have knowledge of theMental Capacity Act and the Mental Health Act. The All Age Continuing Care team are asupportive team with a wealth of skills and expertise providing a robust peersupport and development programme in line with the CHC Competency Framework.There is also a strong focus on the wellbeing of staff. The service operates Monday to Friday betweenthe hours of 9-5 and we offer a home / office hybrid working and flexibleworking opportunities. Due to the nature of the role, there is arequirement to travel, and a full driving licence is essential. The successfulcandidate will also be the subject of an Enhanced DBS check.,
  • Educated to degree level in relevant subject or equivalent level of experience working at a similar level in specialist area, ideally Personal Health Commissioning
  • Nursing qualification or Allied Health Professional Occupational Health, Physiotherapy etc. and registered with a professional body
  • Knowledge of NHS Personal Health Commissioning and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Personal Health Commissioning and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care
  • Evidence of Continued Professional Development
  • Desirable
  • Extensive knowledge acquired through experience and training plus further knowledge of the Continuing Healthcare process
  • Good understanding of Personal Health Commissioning
  • Experience of working in a health environment
  • Negotiation and conflict management skills and the ability to influence
  • Previous experience of caseload management

  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
  • Ability to analyse very complex cases where material is conflicting and drawn from multiple sources
  • Effective communication skills
