Full Job Description

We are seeking a Senior Young Carers Practitioner who will help lead our service offer to young carers and their families, alongside a team of practitioners to ensure high standards of delivery and impact. Family Action's Young Carers Service in County Durham, known locally as The Bridge, provides support to Young Carers, Young Adult Carers, and their families. Family Action have delivered the Young Carers service since 2011 and are an established provider within the sector. We operate a whole family approach to working, with the child, young people or young adults being at the center of the support. The service is commissioned to carry out Young Carer Needs Assessments with Young Carers within County Durham. The service supports families through one-to-one sessions, home visits, school, college or university visits, telephone support and small peer support sessions. Additionally, the service will delivers age-appropriate, focused events and support groups to provide peer support and learning with partner agencies. Main Responsabilities

  • Completing Young Carers Needs Assessment or Young Adult Carers Assessment which will inform a package of support which is tailored to the young person and their family's specific needs. Delivering that support in a flexible and effective manner.
  • Line managing 2-3 Young Carers Practitioners within the team and deputising for the Service Manager where appropriate
  • Managing the waiting list for the service, so that families are safe whilst waiting for our support to begin

    Enthusiasm, a creative approach to work and an ability to engage positively with a wide range of people and ages.
  • Knowledge of a young carers and their needs
  • Experience of previously managing, supervising or mentoring staff/volunteers
  • A sound knowledge of child/young adult development, safeguarding, and the assessment process
  • Qualified to a level 4 or above with a qualification in social work, health, education or a similar field, or equivalent experience of work in this sector.
  • Excellent communication skills.
  • Experience in lead professional roles is desirable.
  • Appointments are subject to Family Action receiving a satisfactory disclosure from the Disclosure and Barring Service.
