Prevention Liaison Officer - Community

An exciting opportunity has arisen for an enthusiastic individual to join our Assistive Technology Team.
You will become part of a small specialist team offering advice and assistance to older, vulnerable or disabled people living in their own homes.
You will complete holistic assessments with an individual for services such as, Assistive Technology, Connect, Handyperson services and other support services for them to remain safe and independent at home.
You will work closely with Social Care teams, managing referrals, completing joint visits and offering advice on what equipment may be required. You will provide information and advice on services which are available to residents of Middlesbrough, family members and professionals.
You will assist with the development and delivery of Assistive Technology workshops for Health professionals and Social Care groups.
A full driving licence is required and the use of your own vehicle. Business insurance is essential.
