Oxford University Hospitals Trust would like to recruit a senior clinician into the role of clinical GP Liaison lead to deliver an interface role between OUH clinicians and Oxfordshire GP's This role will require to be supported by 2 PAs or 8 hours per week. The role has added prominence as the challenges and value of a functioning interface, between primary and secondary care, is increasingly valued. This year the role has been instrumental in securing additional funds for educational materials to support understanding and responsibilities of roles in primary and secondary care and been part of the first NHS Confed Interface Collaboration Project.Main duties of the job Key Relationships: · Oxfordshire GP practices · Oxford University Hospitals clinicians · Berkshire Oxfordshire, Buckinghamshire Integrated Care Board, (BOB ICB) key staff members e.g. Directors, Clinical leads, Heads, Communications team · OUHFT key staff members e.g. Chief Officers, Directors, Communications team · Users and potential users of services and public stakeholders · Oxfordshire County Council and Public Health · Local Medical Committee (LMC), · Berkshire Oxfordshire, Buckinghamshire Integrated Care Board, (BOB ICB) key staff members e.g. Directors, Clinical leads, Heads, Communications team · OUHFT key staff members e.g. Chief Officers, Directors, Communications team · Users and potential users of services and public stakeholders · Oxfordshire County Council and Public Health · Local Medical Committee (LMC),
- This role will lead in building relationships between OUH clinicians and Oxfordshire GP's and other health professionals as mandated in the 2024/25 NHS England Planning Guidance;
- The role will support the development and activity of the newly established Oxfordshire Primary & Secondary Care Liaison meeting, a quarterly meeting providing a shared space for colleagues to find solutions to mutual challenges.
- This role will rapidly develop a robust communication plan to ensure GP practices and other organisations are informed and have the ability for two-way communication via a single point with the resultant necessary action;
- The role will creating positive working relationships and positive service experience for services user, carers and other professionals;
- The role will provide update briefings to divisional directors and operational forum group;
- To work in a collaborative manner within the clinical divisional management structure of the OUH, GP practices and clinical commissioning group; · Ensure timely and effective communication between the Primary Care and secondary using a range of media to support · Meet monthly with Local LMC and BOB LMC building relationships, understanding of key issues and challenges, working collaboratively to offer solutions; · Meet regularly meet with Chief Officers and Divisional Directors; · Develop/maintain links with key external organisations;
- Develop/maintain links with specialties within the Trust;
- Support development in process and pathways that support better patient experience in areas such as outpatients and discharge
- Support vulnerable patients avoiding hospitalization, giving GP feedback, communication with primary care teams;
- To work with the Divisional Directors, Clinical Directors and Operational Service Manager, play an active role in ensuring the delivery of the key actions to improvement communication with primary care.
Highly organized; - Excellent with people, with active listening and communication skills;
- Familiar with how primary care is delivered in the UK and Oxfordshire in particular;
- IT literate;
- Excellent analytical skills, including data extraction, analysis and presentation;
- Target driven;
- Confident and professional;
- Works with in the Oxford University Hospital Trust Values of Learning, Respect, Delivery, Excellence, Compassion and Improvement., 5 years experience, Clinical qualitifcation Desirable criteria
- Familiarity with local, regional and national position / guidance on primary and secondary care interface, Ability / willingness to work out of core hours, You must have appropriate UK professional registration.
