Primary Care Liaison Practitioner

We have an exciting opportunity for a Primary Care Liaison Practitioner in our Richmond Wellbeing Service.

The Richmond Wellbeing Service is committed to work in partnership with service users and their carers, as well as with other statutory and non-statutory services. Our aim is to provide strengths based and recovery orientated interventions in order to maximize the quality of life and wellbeing of service users.

The post holder will work exclusively within the Primary Care Liaison Team (PCL) in the London Borough of Richmond. The PCL team is one part of the Richmond Wellbeing Service, working alongside the psychological therapies team, provided within the IAPT Talking Therapies framework., The post holder will promote clinical excellence and expertise in providing care for patients with mild to moderate mental health difficulties and support the delivery of an agreed and coherent, collaborative and co-produced care plan, in keeping with care pathways, operational policies and NICE guidelines. The post holder will deliver services to maximise engagement of service users and support them in achieving their recovery goals. The post holder will be involved in service development and implementation of the requirements of the NHS Long Term Plan and the National Service Framework for Mental Health Standard 2.
