Full Job Description

We are looking for Mental Health Practitioners to work within Hartlepool One Life Primary Care network and North Stockton Primary Care Network (PCN) . The Mental Health Practitioner will work closely with the GP practice staff, their TEWV colleagues, other agencies and will be working especially close with the new Additional Roles Reimbursement Scheme (ARRS) Workers., We have an exciting opportunity for a Mental Health Practitioner to working with a Primary Care Setting. Main duties of the role will include;

  • Work with patients who have a severe mental illness and/or complex emotional needs who do not meet the criteria for secondary care services and are too complex for IAPT services to support shared decision-making about self-management; facilitate onward access to treatment services; and provide brief evidence-based psychological interventions, where qualified to do so, where the intervention can be safely offered outside of a multi-disciplinary mental health team

  • Work closely with other PCN-based roles to address wider patient needs, e.g. PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support

  • Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate

  • Provide a consultation, advice, triage and liaison function, supported by the local community mental health provider through robust clinical governance structures to maintain quality and safety, including supervision where appropriate

  • Supervision, training and personal development are a key aspect of the post, along with regular supervision/appraisal which we see as an important aspect to the role.

    The post holder will need to have significant experience working with patients with mental illness and will be responsible for short-term treatment of patients referred to you in GP setting. Ideally you will have skills in delivery of the front end interventions to aid patients stay within a primary care setting.


  • Candidates must have 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
