Full Job Description

The Primary Care Mental Health Practitioner role is an exciting development within our primary care mental health workforce. This role will give the post holder the opportunity to apply and use their skills within a primary care setting ensuring that individuals receive appropriate and timely assessment and interventions, while working with partner agencies to embed the community mental health transformation model., The Primary Care Practitioner role is new to Gateshead Inner West and looks to offer a mix of first contact triage and follow up brief interventions. This is an autonomous role and the successful applicant will have access to peer support via a daily huddle with other Primary Care Mental Health Practitioners.
The successful applicant will be offering assessment, brief intervention and navigation to local services to both working age adults and older patients. You should have extensive clinical knowledge and experience across a range of mental health settings. You will be working into the 5 GP surgeries across the PCN (Chainbridge, Teams, Glenpark, Whickham and Sunniside.

Based in the Inner West of Gateshead our Primary Care Network serves over 45,000 patients across our five member Practices of: Whickham Medical Cottage, Chainbridge Medical Partnership, Glenpark Medical Practice, Teams Medical Practice & Sunniside Surgery.
We provide integrated services to our local population. Aiming to deliver more personalised, coordinated health and social care for people close to home.
