Full Job Description

The 3Sixty Care Partnership is a unique joint venture between a federation of 27 GP practices in Kettering, Corby, Wellingborough and East Northants and Northamptonshire Healthcare NHS Foundation Trust. 3Sixty are working together with the five Primary Care Networks (PCNs), by bringing together health and care professionals to support them to deliver joined-up services, which we believe will lead to better quality care, better patient satisfaction and better outcomes., The roles involve working in a multi-disciplinary team in a patient-facing role as well as providing primary support to general practice staff with regards to prescription and medication queries. The successful candidates will take responsibility for areas of chronic disease management within the practice and undertake structured medication reviews to proactively manage patients with complex polypharmacy. In addition, the clinical pharmacist will provide support with medicines optimisation, medicines safety, quality improvement, staff training and manage some aspects of the quality and outcomes framework and enhanced services., Applications will be transferred to the TRAC system, by completing an application you are giving authorisation for the transfer of your data. Applicants will also need to demonstrate a commitment to learning and development and an ability to adapt to different working environments.
