Full Job Description

The roles involve working in a multi-disciplinary team in a patient-facing role as well as providing primary support to general practice staff with regards to prescription and medication queries. The successful candidates will take responsibility for areas of chronic disease management within the practice and undertake structured medication reviews to proactively manage patients with complex polypharmacy. In addition, the clinical pharmacist will provide support with medicines optimisation, medicines safety, quality improvement, staff training and manage some aspects of the quality and outcomes framework and enhanced services.

Northamptonshire Healthcare NHS Foundation Trust (NHFT) on behalf of 3Sixty Care Partnership, and its member practices are excited to announce that they are seeking to recruit motivated and enthusiastic Clinical Pharmacists, with excellent communication skills to join our forward-thinking team to support the safe and effective prescribing of medicines and deliver the Primary Care Network Contract Directed Enhanced Service (DES)., Interested applicants will need to have completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council, as well as experience of working in a pharmacy setting. Applicants will also need to demonstrate a commitment to learning and development and an ability to adapt to different working environments.
