Full Job Description

As a Primary Care Nurse, your responsibilities will vary. You will

  • Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions

  • Undertake medication rounds

  • Dispense controlled medication

  • Attend emergency response situations

  • Assess patients

  • Undertake reception screening.

  • Skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.

    Registered nurse Adult NMC

  • Evidence of Continued Professional Development

  • Understanding of the complexities of multi-disciplinary and multi-agency working

  • A non-judgmental and compassionate approach

  • Excellent communication skills

  • Understanding of clinical governance, health and safety, QOF, EBP and national standards

    The shift pattern is Monday - Sunday hours per week based on a rota basis, covering the following shifts : Monday - Friday - , - and - Weekend - -, This position is full time, youll receive an annual salary up to £38,per annum FTE - depending on experience.
