Primary Care Nurse - HMP & YOI Feltham

The post holder will be Registered Adult or Children's Nurse, working as part of an excellent primary care team, carrying out a range of clinical activities in a secure environment where the young person/young adult is put first.

The post holder will often be the first point of contact for the healthcare needs through initial assessment, screening and/or triage of young people/young adults in accordance with specific standard operating procedures. The successful candidate will assess, plan and implement care for patients within each care/treatment pathway in order to promote wellbeing, facilitating the use of evidence-based practice to ensure clinical care is both effective and appropriate to the young person/young adult.

The post holder will carry out general nursing duties, maintain contemporaneous records, support their colleagues and make a professional contribution to the development of an integrated healthcare service.

The post holder will also act as a preceptor/mentor according to experience.

1. Registered (NMC) nurse Adult or Children's Nurse
2. Numeracy skills required for the safe administration of medicine
3. Previous experience of working in Primary Care services either as a band 5 nurse or a student nurse
