Full Job Description

We are looking for an enthusiastic and dynamic team leader to work across our primary care Mental Health team in Southampton. This is an emerging team that has grown and developed over the last three years in line with the national transformation programme for Mental Health Services. The post holder would be expected to work across the seven primary care teams in Southampton and would provide operational and clinical leadership capacity as directed by the primary care service manager. You must be a registered pracitioner (Mental health nurse, social worker or Occupational therapist) with expereince of working clinically within a mental health setting and evidence of leading people., To support the primary care service manager in providing transformational leadership across the Enhanced Mental Health Primary Care teams in Southampton.

  • Under the direction of the primary care service manager work collaboratively with Primary Care Networks, Providers, Voluntary and Third Sector Agencies to ensure services are accessible, pro-active and can provide a full range of interventions based on the principles of recovery and empowerment.
  • Under the direction of the primary care service manager provide focused clinical input into a team where need arises.
  • Under the direction of the primary care service manager support audit and evaluation of the Enhanced Primary Mental Health Care Model through active collaboration with all partners involved in the Enhanced Mental Health offer.
  • To be compassionate in meeting the needs of clients, their carers and families.
  • To provide short term clinical care into defined teams where required to do so.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
