Primary Care Transformation Lead

We are excited to recruit into an exciting new position of Primary Care Transformation Lead, working closely with our general practices, Primary Care Networks, GP federation, ICB Primary Care team and wider health and care partnership across Greenwich., This is a significant task in the context of rising pressures on the NHS and in our communities more widely. Working closely with our colleagues in the Healthier Greenwich Partnership and across the South East London health and care footprint, we are building our Primary Care team capacity to deliver against ambitious plans over coming years.

The NHS South East London Integrated Care Board is an NHS statutory organisation. We bring together partners involved in planning and providing NHS services, working together to meet the health needs of the population within south east London and deliver the Integrated Care Partnerships strategy. We are responsible for allocating NHS resource to deliver this plan.
