Full Job Description

a) To play an integral role in the Greenwich Primary Care team, working closely with all team members as a well-communicating and mutually supportive team who aspires to high quality and efficiency standards in how we work. b) To work closely with Greenwich Health (Greenwichs GP federation) on areas of work that include supporting general practice transformation and sustainability, and supporting their role as the training hub for Greenwich. c) To develop collaborative, mutually productive relationships with Greenwich general practices, Primary Care Networks and the organisations that support our practices and their patients. d) To be mindful of the role of Primary Care within the Healthier Greenwich Partnership (Greenwichs Local Care Partnership) and to develop good working relationships as required with colleagues from partner organisations such as Greenwich Council, Oxleas, Lewisham and Greenwich NHS Trust, HealthWatch, Bexley and Greenwich Hospice and the local voluntary sector. e) To
lead or support primary care improvement initiatives, service or pathway redesign and procurements, to help deliver the transformation that we aim to achieve in Greenwich and South East London over the next five years. This will include stakeholder engagement, project design, options analysis, project progress management, risk management, sound financial management (including value for money), outcomes and performance reporting and evaluation. f) To participate in patient, public and carer engagement as required, through a range of different engagement and communications methods and analysis or follow-up afterwards as appropriate. g) To keep abreast of health and care related policy development and initiatives from NHS England, the Department of Health, the Care Quality Commission and others, and contributing to South East London and Greenwich strategy and planning as required. h) Deputising for colleagues and leaders in the Primary Care team or more widely as required, and representing
Greenwich borough or South East London at external meetings and events as required. i) Actively considering the health and care inequalities experienced by our population and reflecting this in the work we do, and adhering to the principles of Equality, Diversity and Inclusion and anti-racism both internally to our organisation and through our work with health providers, stakeholders and patients. j) Carrying out other duties as may reasonably be required by your line manager to deliver the business of the Greenwich Primary Care team for the benefit of local partners and residents. There are currently no direct line management responsibilities within this role.
