The Adult Community Teams are part of the wider Care Community Partnerships with GP's, Advanced Clinical Practitioners, Mental Health services, Housing and voluntary sector services as they take a collaborative approach to supporting people within its footprint. The Community Teams provide support for any adults over the age of 18 years and includes, complex adult safeguarding, those with a learning disability, autism, mental health concerns, physical disabilities and homelessness. This is an exciting opportunity for experienced Social Workers which requires the use of all the core skills, knowledge and Social Work values including good communication, decision making skills and the ability to work under pressure. The successful applicant will need to be able to adopt a flexible approach to their work and be a good team player. The post holder will be expected to be able to undertake person centred and strengths-based assessments either within the individual's own home, hospital or Care Home, with the aim of improving client outcomes for the residents of Cheshire East. Additionally, this post will provide a wide range of learning and training opportunities and includes joint working across the Health and Social Care sector. The area in which the post holder will be expected to travel for visits is Knutsford however there will be times when the post holder will be expected to travel to Macclesfield Town Hall for meetings. Cheshire East Council (CEC) is looking for experienced and self-motivated Grade 9 Social Worker to join our innovative and friendly Adult Community Team.,

  • We are looking for enthusiastic and experienced Social Workers who have good knowledge of adult safeguarding, mental capacity and sound Best Interest decisions making including knowledge of Court of Protection issues.
  • The candidate should be self-motivated with excellent communication skills and the ability to work both autonomously within the Adult Community Teams and wider Multi-disciplinary arena. Organisational skills and time management is key to effectively managing caseloads.
  • The candidates should be able to provide a client centred approach to care and demonstrate sound knowledge through evidence-based assessments and management of support in a variety of situations.
  • It will be essential for the post holder to work autonomously within the Council's policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
