Full Job Description

We are seeking an experienced, enthusiastic & delivery focussed project manager to help us set up and deliver Hywel Dda's Six Goals Programme. This is a innovative and exciting programme of improvement projects spanning across Urgent and Emergency, Secondary, Primary and Social Care . The Six Goals Programme is leading Wales in the development of Urgent and Emergency Care (UEC) pathways which will transform the system flows and pathways in which Health and Social Care operate. The programme will develop and implement initiatives and projects across Hywel Dda in line with Welsh Government's six policy goals for UEC: Coordination, planning & support for high-risk groups, Signposting to the right place first time, Access to clinically safe alternatives to admission to hospital, Rapid response in a physical or mental health crisis, Optimal hospital care following admission, and Homefirst approach & reduce the risk of readmission., As a Project Manager within the team you will:

Lead on a range of strategic service improvement projects, service developments and capital projects as part of the delivery of the Health Board's Six Goals Programme. The projects will encompass service delivery improvements, partnership initiatives, commissioning projects, including qualitative and quantitative improvements and environmental (estate) improvements. Contribute to the development of the Health Board's Annual Plan/Integrated Medium Term Plan.

Work with Urgent Emergency Care (UEC) leads, clinicians and staff across the Health Board and wider organisations, and will promote innovation and best practice in bringing about service change in support of national priorities and drivers and responsive to local health need.

Provide support and advice to managers and clinicians in the Health Board on project delivery of the modernisation agenda to bring about a modernised, patient focused service.

Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Five community hospitals: Amman Valley Hospital and Llandovery Hospital in Carmarthenshire, Tregaron Hospital in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire.

Two integrated care centres (Aberaeron and Cardigan, Ceredigion).

Community facilities, including:

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
