Full Job Description

The Cheshire and Mersey Mental Health and Community services Provider Collaborative brings together the NHS Providers of Mental Health, Learning Disabilities and Community Services in Cheshire & Mersey to work collaboratively to deliver better care for our citizens. The Collaborative is hosted by Bridgewater Community Healthcare NHS Trust.

The Collaborative seeks to identify and realise the opportunities that exist from Organisations working closer together, to share best practice and to provide support to each other to deliver the best services.

Our core purpose is to deliver connected pathways of care for people in their communities, especially those with multiple physical and mental health needs, where a system-wide approach is needed to achieve the best outcomes.

Main duties of the job

To support the coordination and delivery of multiple major change or redesign programmes within the remit of the MHLDC Community Urgent Care Programme.

The post holder will be responsible for co-ordination and management of a projects(s) including providing assurance, tracking and monitoring of implementation and benefits realisation as appropriate supporting and enabling effective change management across the system.

Flexible working will be considered for all roles.
