Full Job Description

Surrey and Borders Partnership NHS Foundation Trust Psychiatric Liaison Services provide high quality mental health care to people in people. If you are looking for an exciting opportunity, our service has expanded and there are vacancies for band 6 psychiatric liaison practitioners. We are based in the acute hospital and is an assessment and signposting service. Our service is core 24 and is accredited by the RCPsych. Our team is dynamic, culturally diverse, inclusive, very supportive and creative. We work collaborative with our colleagues in the acute hospital. We value feedback from our service users and family/carers. This role attracts a local Retention Premium of £500 payable annually after 3 years of Service in the Team and each year thereafter as well as Recruitment Premium of £3,000 payable on appointment.,

  • To utilise a range of specialist skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen
  • To co-ordinate and monitor the delivery of person centered healthcare to a designated group of people who use our services, using complex clinical decision making where appropriate.
  • To be responsible for the full assessment of the psychiatric/psychosocial needs of those patients who have been referred to the service; including the comprehensive assessment of risks to self and others
  • To be responsible for the development, implementation and evaluation of robust discharge plans based on a clear assessment and formulation of the person's needs; which may include urgent referral to secondary mental health services, social and voluntary services
  • To work as a member of the MDT in assessing and planning care and responding to changes in people who use our services
  • To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.
  • To ensure provision of therapeutic activities, emotional support and stability to clients to ensure effective engagement whilst maintaining professional boundaries.
  • To create, develop and maintain professional supportive relationships with all members of staff, with other professionals and agencies to enhance recovery.
  • To act as role model in providing a service for people with mental health problems within hospital setting
