Full Job Description
We are seeking an experienced and compassionate Practice Nurse to join our dynamic primary care team at Mill Hill Surgery.
As a key member of our multidisciplinary team, you will provide high-quality nursing care to our patients, promoting health and wellbeing in the community.
The ideal candidate will have a broad range of clinical skills, excellent communication abilities, and a strong commitment to patient-centered care.
Our practice, Mill Hill Surgery in Ealing / Acton, is composed of six (6) GP partners, along with a Nursing team including Nurse Associate support, a newly appointed GPA role, and a comprehensive admin support team providing primary care services to a registered population of approximately 7000 patients across the borough of Ealing.

As a GP Training Practice, we offer excellent support, supervision and training to support ongoing professional development., General Nursing Duties:
+ Assess, plan, implement, and evaluate patient care in line with clinical guidelines and best practices.
+ Carry out basic wound care, dressings, and management of minor injuries, provide health education and advice, promoting self-care where appropriate.
Chronic Disease Management:
+ Provide care and support to patients with long-term conditions (Asthma, COPD, diabetes, hypertension, and cardiovascular diseases).
+ Offer lifestyle advice and coordinate care.
Immunisations-Vaccinations:
+ Administer childhood and adult immunisations as part of the vaccination schedule and provide travel vaccinations and advice on travel health.
Health Screening and Assessments:
+ Carry out health checks for patients, including blood pressure monitoring, phlebotomy, and weight management, conduct cervical smears and sexual health screening.
Minor Illness and Injury Management:
+ Offer wound care and first aid, including assessment of minor injuries and referral when appropriate.
Patient Education:
+ Educate patients on disease prevention, healthy living, and self-management of conditions
Administrative Duties:
+ Maintain accurate, up-to-date patient records, in line with legal and confidentiality requirements and participate in audit and quality improvement initiatives to enhance patient care and practice performance.
Multi-disciplinary Team Collaboration:
+ Work closely with GPs to deliver integrated patient care.
