Full Job Description

To work autonomously taking responsibility for care, quality, and clinical activity for a designated caseload/team. To contribute to care quality performance and clinical activity by providing professional nursing/AHP leadership and guidance, clinical advice, and management in relation to the key dimensions of health care quality:

  • Patient safety
  • The effectiveness of all health care interventions
  • Patient experience
  • The environment in which care is delivered
  • To participate in the effective clinical leadership and management of the team. To actively contribute to the improvement of service and quality of care provision to meet the needs of patients, service users and their families. To act as a role model, mentoring, teaching, assessing and supporting students and learners on placement and participate in the education, development and mentorship of other staff. To work collaborate with other senior nurses/AHPs across LPT as appropriate and promote the role of their speciality nursing/AHP to all stakeholders. To foster a culture of networking in an open and transparent approach with internal and external partnerships and between all stakeholders. To be responsible for the assessment, planning, implementation and evaluation of programmes of evidenced based care to a group of patients according to changing healthcare needs and varying levels of complexity., working patterns in line with our flexible working policy. For all substantive roles, new staff (excluding medical staff) are appointed subject to a 6-month probationary period (see Probation Policy). All jobs will require permission to work in the UK.     Applicants will hold registration with the NMC or HCPC and be motivated and enthusiastic with excellent communication skills. Experience in respiratory/cardiac/long covid management of patients is important/desirable, along with in depth knowledge around COPD, Heart Failure and Long Covid. Support and training will be given to the successful applicant to upskill in this area if needed. Experience of delivering Pulmonary/Cardiac/Long Covid Rehabilitation and/or community working is desirable. The successful applicant will work within a multi-disciplinary team and be supported by a Band 7 Team Lead and Clinical Specialists. The post is permanent either full or part time working. It will involve travel across the County to provide rehab in several locations and having a valid UK driving license and access to a vehicle is essential to fulfil the job role.
