Full Job Description

A qualified nurse who will support the team lead in ensuring that palliative patients receive the highest standard of care. To act as a role model and support the team manager to operationally manage and lead a team of carers who provide care to patients in their own homes. To be proficient in IT skills and to ensure the management of rotas and support the RPCS team manager with monthly reporting requirements with accurate recording of data. Main duties of the job To support the team lead in clinical assessment of patients and the management of the caseload. To support and act as a mentor for carers and liaise with members of the health and social care professional, patients, families and carers. To ensure accurate recording of data, management of daily rotas and to be part of the out of hrs call rota. To work flexibly across the organisation. Acting as a Rennie Grove Peace hospice ambassador within the community., To work in the community carrying out clinical assessments with regards to holistic care needs of patients and their families. To work alongside the RPCS carers, providing support, mentorship and be responsible for signing off competencies. To manage the rotas and input data to ensure the smooth running of the service.

  • Registered General Nurse with relevant professional qualification.
  • Experience of clinical assessment.
  • Understanding the importance of confidentiality and Information Governance.
  • Willingness to undertake personal development to improve skills associated with job role
  • A good understanding of technology with a working knowledge of Microsoft office packages Word, Excel and Outlook.
  • Able to build good working relationships.
  • Able to communicate effectively both verbally and in writing.
  • Good administrative and record keeping skills and able to work with good attention to detail.
  • Strong organisational skills.
  • Sensitivity and understanding to the needs of volunteers and customers, many of whom may have experienced Rennie Grove Peace services or be bereaved themselves.
