Full Job Description

This role will report to the Team Manager, this is a role working collaboratively with young people to understand their goals, identify which areas of life are challenging and then working together to improve wellbeing. You will be providing a professional social work service to young adults, their families and carers in line with the Care Act 2014 as they transition to adulthood.

With a particular focus on neuro-divergence you will be working in a way which respects the individual's skills, interests, preferences and difficulties, proactively working in partnership with the individual, the people who are important to them and a range of professionals, including from children's services, education and health.

Keeping the young person in control, you will be working with them to understand and explain their strengths and their needs helping them to achieve their goals as they move through adolescence and into adulthood.

You will work with them to identify and achieve their goals as they move through adolescence and into adulthood and will be a named point of contact offering continuity and familiarity to the young adult and those in their circle of support.

A Qualified Social Worker, registered with Social Work England or a Qualified Occupational Therapist registered with Health and Care Professions Council.

Demonstrable experience of working in partnership with individuals with a range of needs to identify their strengths, support needs and goals, and making plans to achieve these goals and have the ability to be flexible and adaptable, tailoring your approach to the needs and preferences of the people you are working with.

Experience of working in a collaborative way and of co-production and have effective and flexible communication skills

A thorough understanding of the legislation that governs adult social care, (Care Act; Mental Health Act; Mental Capacity Act; Autism Act etc) and a comprehensive awareness of practice to safeguard adults. You will be fully conversant with legislation and guidance and demonstrate that you implement this in your day-to-day practice.
