Full Job Description

HWICB are seeking an experienced, highly motivated, flexible and multi-skilled Registered Nurse/ associated Allied Health Professional/ Social Care Practitioner with excellent communication and organisational skills as the new Governance, Quality and Partnership Lead for Continuing Healthcare. This role is a vital part of the All Age Continuing Care Team and an integral part of our dynamic Continuing Healthcare (CHC) team within the ICB. The successful candidate will be a Senior Clinician/AHP/ Social Worker with significant experience of Continuing Healthcare and will act as line manager for a small team. The post holder will be responsible the development of the CHC service as well as multi-agency and stakeholder engagement which aims to enhance working relationships, strengthen policies and processes and provide assurances in relation to Continuing Healthcare. We encourage interested candidates to apply at your earliest convenience to ensure your application is considered., In addition to supporting the operational function of CHC in line with the National Framework for CHC, the post holder will work together with multi-agencies, stakeholders/ other professionals across both the public and private sector, to ensure practice standards and policies are in place which uphold the ICB's statutory responsibilities in relation to all aspects of CHC. The post holder will provide assurances to the CHC leadership team that in collaboration with Local Authority Partners, the processes set out within the CHC National Framework are communicated, and compliance is monitored across the health and social care sector. The post holder will actively contribute to the strategic planning of team projects, identifying interdependencies across projects and functions along with potential impacts on the wider integrated care system (ICS). They will be pivotal to ensuring that the CHC service meets the needs of all age groups to improve experience and outcomes.

If you are a nurse, you will be registered with the Nursing and Midwifery Council as a Registered Nurse. If you are an AHP or Social Worker you will be educated to a degree level or equivalent and, in all cases, you will have appropriate experience in the provision of care across all ages and have experience in CHC. Driver's licence and access to own vehicle with business insurance is essential.
