The post holder will be responsible for leading safe, resilient, and consistent team coordination and collaboration in facilitating safe and effective discharge in line with Trust policies and procedures. This includes supporting the planning and coordination of patient flow across the care pathways within the Division. The role involves collaboration with ward teams, key partner organisations, service users, families, carers, and other key agencies to ensure meaningful admission and safe, appropriate discharge. As a senior clinical leadership role, it focuses on the quality of the patient experience and journey through the delivery of high-quality, safe, and effective care. We are looking for a highly motivated Quality Improvement Nurse Manager for Patient Flow to join our busy, friendly and supportive Division. You will support the Division to ensure challenges are given responsive attention to effectively ensure the patient flow is maintained, engaging with both our internal and external stakeholders. This role is vital in the delivery and quality improvement of effective patient flow, support to the Division MDT and Senior leadership teams to perform to a very high standard. We require someone who is a team player, with excellent communication skills and who visibly enjoys the challenges that the QI Clinical Manager for Patient Flow role provides.
