The key aims of the service are to: reduce urgent admissions and attendances to hospital from care homes; reduce unnecessary reactive demand on GPs, and; provide a seamless, high quality and integrated approach to caring for people in care homes. The Quest Nurse will work within a Multi-disciplinary team whose core membership will include:
Community Matron(s)
Consultant Physician for complex Elderly
Community Pharmacist
Therapist (Occupation therapist or physical therapist)
Community older people's mental health professional

This as an exciting time for our continuing development and we are ensuring that the organisation is fit for purpose by introducing and embedding new skills and behaviours., Provide high quality, comprehensive, holistic and timely assessments and treatments for patients and their families under the guidance of the Quest Matron, using an assessment process to achieve patient focused care at all stages of their care including palliative stage.
Negotiate goals for lifestyle modification.
Be proactive in health promotion, provide patients and carers with information and advice about their condition ensuring the best level of understanding and promote self-care and management at all times.
Utilise counselling skills to support patients, carers and staff, whilst remaining aware of limitations and other specialist resources available.
Maintain clear, concise and up-to-date clinical records, in line with NMC Record keeping guidance
Maintain and develop relevant expertise and knowledge related to the intricacies of caring for patients with complex health needs
Ensure open channels of communication with both internal and external agencies, to achieve a coordinated service to patients and their families.
Demonstrate a high level of interpersonal skills, self-awareness and empathy in all communications.
To provide education, training and support to the nursing and care home team and ensure the highest standards of care are provided for patients.
Assist the care home staff in the delivery of planned packages of care working to evidence based care plan
Undertake delegated care without direct supervision where appropriate. This may require the use of observational/ analytical skills to monitor the health and wellbeing of clients.
Observe a personal duty of care when using equipment ensuring own safety and that of others.
Contribute to the management of aggressive/ abusive behaviour
Professional Duties
Act as a role model for quality nursing care at all times, promoting the need for effective evidence based interventions and compassionate care.
Act as patient advocate at all points of the care pathway.
Work within the NMC Code of Conduct at all times.
Adhere to national, local and professional standards, guidance and directives relevant to their sphere of practice.
Work collaboratively with the commissioners and other providers to ensure a consistent approach to service development.
Promote the service through communications and media internally
Frequent contact with staff at all levels including senior clinicians through the delivery of training
Verbal and written communication on data and reports
Formal presentations to teams and groups
Develop and foster effective support networks locally, regionally and nationally and attend appropriate meetings relevant to role. To maintain links with all relevant agencies.
Undertake on-going service review which will shape the service in response to user involvement, professional opinion and audit / survey data along with local and national guidelines/evidence.
Ensure electronic record is updated in real time and that data is validated and assured
Adhere to CHFT policies and protocols.
Audit work to aid service performance and development
Establish and maintain effective communication pathways including the use of IT systems with relevant health and social agencies and professionals.
Communicate relevant and factual information to patients and carers using sensitivity and tact.
Report any changes to a patient's condition to the appropriate member of the team at the earliest opportunity
Ensures effective communication is maintained for those patients with communication difficulties.
Managerial Duties
Ensure adherence to all CHFT protocols, policies and directives within service delivery.
Participate in service review and development alongside the directorate team.
Work with other appropriate professionals to ensure holistic, comprehensive nursing assessment, care planning, care delivery and referral mechanisms are in place to support patients at appropriate points of their care pathway, ensuring advice and information is available to both patients and carers in all care settings.
To ensure that continuity of care for patients and appropriate specialist nursing or other health and social support is available and accessible as appropriate, liaising where appropriate with other care settings and professionals.
Support CHFT's aims and objectives around service delivery and achieving targets.
Work closely with the appropriate nurses, matrons/community nurses/Allied Health Care professionals to influence the nursing service and care provision for patients in nursing and care homes
Work in partnership with clinical leads and appropriate members of the MDT to review protocols and policies and help develop new ones where appropriate.
Work with the identified clinical leads and appropriate members of the MDT to support the reviews of all patient/carer information and the production of appropriate new information as identified, ensuring these are offered at all appropriate points of the pathway of care in line with local, network and national guidelines and initiatives as appropriate.
Within the framework of clinical governance, work continuously to improve quality and safeguard standards of care, maintaining a patient focused service, promoting and implementing evidence based practice with measurable standards of care and outcomes. Have involvement in audit projects to measure standards of care. To identify and highlight any gaps in the evidence/service provision for patients in nursing and care homes.
Attend regular relevant meetings (internal and external) and provide effective channels of communication within the team as required.
Maintain a flexible approach and develop the role according to needs.
Participate in and promote patient and public involvement in the QUEST service
Develop relevant educational resources for patients, carers and staff
Financial Duties
There are no financial duties required in this post.
Managing self
+ Participate in regular supervision.
+ Attend all mandatory training.
+ Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal.

The Trust will administer the DBS check on your behalf and will recover the cost (Enhanced £42.90, Standard £22.90 or Basic £22.90) from your salary when you commence in post (including Internal staff). You will also be required to participate in the DBS Update Service and pay the £13 cost per year. This is a condition of your employment.
