Full Job Description
Southend City Council are looking to appoint a permanent, full time, mental health experienced Social Care Assessor.
Southend have a newly formed Adult Mental Health Team and they require the Social Care Assessor to undertake Care Acts and Mental Capacity Assessments, review existing social care packages and Court of Protection Reviews.
You will champion supporting people with specialist needs to have a fulfilling life. Working closely with all partners (such as the police), you will help to ensure that there is effective coordination of the resolution of risks and other issues that may arise. You will need to apply a strength based approach to practice, both in terms of individual assessments, care plans and reviews.
The successful candidate may be needed to work outside the core hours, weekends and bank holidays when required, including having some on-call responsibility when needed.
Within this position, hybrid working opportunities are available. However, you are required to be onsite at the Civic Centre a minimum of 2 days a week.
