Full Job Description

  • On top of the 5.6 weeks annual leave with Age UK Plymouth your annual leave will also increase by an additional day with each year of service for the 1st 5 years.

    This post involves providing a commissioned service as agreed with the NHS at the point of discharge for individuals living in their own homes. It will involve a variety of different task which may include, collection of patients from hospital, helping them to get to appointments, basic food preparation, light cleaning, shopping and collecting prescriptions for them.
  • Successful individuals will be compassionate, organised and highly motivated to provide high levels of support to clients, ensuring that those initial weeks post discharge pave the way to returning to living independently at home. No personal care is required. Previous experience in a similar role is desirable but full training will be given. It is essential that the successful candidate has a clean driving licence and access to a vehicle for work purposes, and a willingness to take clients in their own vehicle to appointments. You will also require basic business insurance. A mileage allowance will be paid (45p per mile). A post of this nature is subject to an enhanced Disclosure Check through the Disclosure and Barring Service.
