Full Job Description
We are looking for an enthusiastic, dynamic and forward-thinking individual with excellent, organisational, communication and clinical skills to join our Urgent Community Response service. You will be required to work as an autonomous practitioner and be a team player as part of larger multidisciplinary team. Our UCR service works with patients in their homes across the Warrington Borough to avoid hospital admissions and support those being discharged home.
The base is currently Irwell Road Clinic UCR, this will be subject to change.
Our multidisciplinary team work flexibly to provide a 7-day service, 365 days a year between 8am and 8pm.
-We deliver rapid response to people in the community who are experiencing acute episodes of ill-health and require complex management in order to prevent hospital admission and support timely hospital discharges. You will have the responsibility for proactively monitoring and managing patients to ensure the best outcomes., To act as a senior clinical professional within the Rapid Response service, as an integrated health and social care team providing an urgent rapid response to Warrington residents, experiencing a health crisis. The aim is to support people to remain at home rather than be admitted to hospital. Liaising with patients the team and colleagues across the wider health and social care environment including NWAS the postholder will promote, maintain and enhance effective multidisciplinary patient care.
To act as an independent autonomous practitioner to provide specialist treatment and advice to patients and staff which will contribute to the assessment, diagnosis, care and treatment of patients at home preventing unnecessary hospital admission. All referrals will be triaged immediately and responded to within two hours, with referrals from NWAS prioritised.
Possess and demonstrate extended clinical skills in the assessment, diagnosis, treatment and management of a wide range of patient care needs. Professional autonomy and clinical judgement will be exercised in managing, referring and signposting patients on to wider community services and assets.
