Rapid Response provides a Countywide service to manage and treat patients who present with an acute illness or exacerbation of a long-term condition. Rapid Response service also delivers a falls pick up service for people experiencing both non-injurious and minor injury falls. Our aim is to avoid unnecessary hospital admissions and facilitate early discharge from the acute hospitals. Our passion for delivering care services of the highest quality to our local communities is further developing and we are striving to deliver new models of service. Rapid Response Clinicians work a range of shift patterns and shift duration is 10.5 hours., · Complete new and follow up activity and deliver planned treatment and/or refer on to other appropriate members of the Integrated Community Team to provide on-going support.
· Working Countywide to support with all aspects of 2-hour Urgent Community response within own level of competence
· Regularly evaluate care plans to ensure they are up to date and appropriate, reporting clinical changes or areas of concern to the appropriate member of the team.
· Provide skilled clinical interventions, support and advice to colleagues, patients and carers.
· Where applicable issue medication under a PGD appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and within the scope of practice and legislative framework.

Salary Update
With effect from 1 April 2024, there is a 5.5 per cent consolidated uplift for all Agenda for Change (AfC) staff on NHS terms and conditions. This may change the salary that is advertised on this advert.
The national NHS Employers team are working through the processes required to implement the recently announced new Agenda for Change pay award for 1st April 2024 to 31st March 2025. The related national Pay Advisory Notice (which confirms the details including dates) is yet to be issued to employers.

Applicants must have relevant post registration clinical experience, cannulation, phlebotomy and IV administration skills are desirable. You should be an adaptable team player and possess excellent problem solving, communication and IT skills. Community experience is desirable but not essential as a full and comprehensive induction program will be provided, A valid UK driving licence plus the use of own transport with insurance for business purposes is essential.
