Full Job Description

The Rapid Response Team is a supportive and friendly team of nurses, paramedics and therapists. We have an exciting opportunity for an enthusiastic and motivated nurse or paramedic to join the community team, based at Hollyfield Road, Surbiton The team is part of the 2-hour urgent response service responding to adults who are experiencing a health crisis and require assessment and intervention to support them in their own home, rather than being admitted to hospital. The role involves assessing patients in their own home, formulating working diagnosis and management plans. It also involves working collaboratively with multidisciplinary professionals in order to manage complex health and social care needs. This is an exciting opportunity to enhance and develop a wide range of skills including physical assessment and clinical decision-making in a community setting.,

  • To respond to urgent community referrals within 2 hours to those adults who are experiencing a sudden deterioration in health or well- being and require crisis intervention to prevent an unnecessary hospital admission
  • To complete a comprehensive holistic assessment and formulate a management plan
  • To develop skills in autonomous practice including physical assessment, clinical reasoning and differential diagnosis
  • To work collaboratively with urgent community response (UCR) therapists and multidisciplinary teams in the management of complex health and social care issues
  • To provide supervision and support to students and junior staff as appropriate
  • To contribute to the rapid response/UCR service delivery ensuring a responsive, high quality and effective service
  • To record accurate legible health records

    The ideal candidate should be able to plan and prioritise their own workload, be confident in independent decision making, with an awareness or experience of physical assessment.
  • The position would suit an experienced clinician who is looking to develop skills in advanced clinical and autonomous practice. Appropriate training, development and support will be provided.
