Full Job Description

We are looking for an enthusiastic, dynamic, individual with excellent IT skills to join our diagnostic team as our cancer navigator/co-ordinator. The Rapid Diagnostic Service/ Non specific symptoms Patient navigator is responsible for ensuring and facilitating an efficient and effective transition for patients on the NSS cancer pathway. The post holder will ensure the patient receives the appropriate diagnostic test and first definitive treatment within the specific timescale. The post holder will ensure excellent patient coordination and support with patients having a single point of contact (the navigator) throughout their diagnostic journey, alongside access to the right information, support and advice. In addition, the post holder with support the CNS team as required with the pre-diagnosis service which is responsible for supporting complex and vulnerable patients on suspected 2 week wait pathways to ensure we achieve the 28 day Faster Diagnosis Standard. As part of the diagnostic support team you will be supporting our GRAIL trial patients within Lincolnshire.,

  • Patient navigator roles may, additionally, support the NSS service by ensuring shared and consistent information about the patient's journey is available to all those involved in the process.
  • Provide an initial point of contact for GPs to telephone if they have questions about the NSS referral criteria.
  • Track patients referred to the NSS to ensure they are meeting the Faster Diagnostic Standards.
  • Coordinate and attend Virtual triage meetings and support the patient on to the next step in their pathway.
  • Ensuring all necessary patients are discussed, and agreed actions are followed up.
  • Contact Patients and support them as directed by the CNS team
  • Working collaboratively with the CNS team to support all aspects of pre diagnostic phase.
