Full Job Description

You will be responsible for the screening and processing of referrals into the Reablement Service from, but not limited to, Early Intervention, Hospital and Community Therapy Teams using our in-house systems.

You will liaise with referrers, people using the service and their families via the telephone daily, including dealing with enquiries for, and processing, assistive technology requests to enable safe discharge from hospital.

You will process Reablement requests to ensure a timely and safe discharge home from hospital or for support at home.

You will be a first point of contact for enquires into the Reablement service, dealing with telephone enquiries in a timely manner or referring on to appropriate colleagues.

You will be a good communicator with excellent customer care skills in dealing with both internal and external customers.

You will have an understanding and empathetic attitude towards people using the service.

You will be well organised whilst ensuring that your work is prioritised and processed in a timely manner.

You will have a proactive approach and be open to future service developments including integration with our health colleagues.

This may involve the provision of equipment, adaptations to the home and linking people in with community activities they enjoy.

Reablement use an integrated approach with our health partners in Wigan Wrightington & Leigh, Clinical Care Group, Bridgewater Trust and 5 Borough Partnership to support people.

Salary: Grade 5, £24,702 - £27,334 per annum. £12.80 - £14.16 per hour
