Full Job Description
As a Community Reablement Co-ordinator you will play a vital role through brokering services, communicating with external providers, colleagues and residents to deliver and maintain a high standard in service provision. Utilising your IT skills will ensure information is accurately recorded on the appropriate systems relating to purchasing of services.
Are you a confident and effective communicator who enjoys working with others? Do you have good IT skills, being able to use different software programmes? Do you thrive in a busy environment and have the skills to work on your own initiative as well as being a team player? If so, then this is an exciting opportunity to join our new service., Note: Applicants appointed to posts that require an enhanced DBS clearance must declare any periods of time that they have lived, studied or worked abroad and be prepared to provide an Overseas Criminal Records Certificate (OCRC) or a Statement of Good Conduct (SOGC) relating to anytime they were overseas
