Full Job Description
The Discharge to assess (D2A) aspires to provide a responsive, safe effective caring service. The service aims to include providing a local point of access, proactive case management and adoption of an integrated approach with patients. The RFT, whilst operating with the IDS hub, delivers a joined up approach across the locality, to deliver excellence in care and engages with all practices and wider stakeholders in the locality.
The Reablement Flow Assessor, in conjunction with a wide range of clinical colleagues and specifically, GP's, Practical Nurses, Social Care professionals, Consultants, Therapists and Ward Nurses will lead and facilitate a patient or client focused, co-ordinated case management approach across the acute sector and community for people who are assessment fit for discharge, to plan their discharge and effect a safe transfer out of the acute sector.
The post holder will participate in the influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patients' own home as possible.
Open only to applicants from HIOW NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service or Hampshire Hospitals FT., The Care Assessor/Reviewer will work as an active member of the Integrated Intermediate Care Hub and in partnership with therapy teams for Southern Health Care Team for Southern Health to provide a high quality, efficient and effect service to patients.
The Assessor/Reviewer will be proficient, knowledgeable and competent. Educated to NVQ level 2/3, or equivalent, they will continue to develop in their role through the acquisition of further skills, knowledge and competences, to be determined by the needs of patients, the service and the team.
