Full Job Description
The Reablement Service is part of the Adults Wellbeing and Health Partnerships directorate. We are a multi-disciplinary team who provide short term support for up to 6 weeks, to people usually in their own homes to promote independence and choice for people by helping them work towards their reablement goals as set out in their support plan. Our aim is to help improve people's quality of life, maximise long term independence and enable people to remain or return to live in their own home within the community. We are an Occupational Therapy-led service and our community workforce includes Occupational Therapists, Specialist Workers, Team Leaders, Integrated Reablement Workers and Reablement Assistants. All roles work in a way that seeks to improve people's lives by working on their strengths, rather than their weaknesses, and making use of the community and networks around them, as well as local authority-provided services. We work closely with some private providers across Surrey who are part of our Collaborative Reablement Service (CRS). CRS is an extension of our Reablement Teams - by having CRS partners we are able to have more capacity to support more Surrey residents. About the Role As a Team Leader you will be responsible for coordinating the day-to-day work of the team, to deliver a high-quality service in accordance with regulatory requirements. This will include triaging all cases needing reablement support and managing a small team of Reablement Assistants and Integrated Reablement Workers to support people to achieve their support plan goals. You will work alongside the Reablement Occupational Therapy team and wider Health and Social Care colleagues in order to deliver the best possible service to people we support. You will work part of your hours on a rolling weekend rota and will work closely with Team Leaders in other areas to support the wider Reablement Service and co-ordinate staff rotas., Our application form and onboarding process will provide an opportunity to request any reasonable adjustments for the interview and/or the role. This role has a starting salary of £32,512 per annum, based on a 36 hour working week. We are excited to be hiring 5 new Reablement Team Leaders to join our fantastic Reablement Service. The posts will be based in: Mid area (Elmbridge) - 1 full time position NW area (Woking) - 2 full time positions SH area (Surrey Heath, Camberley) - 2 full time positions (these roles will involve providing support to both the Day and Night service) The roles will include both office-based and remote working.
