This is an exciting opportunity for experienced health care staff to join the team in Longmoor House., The Reablement Worker will function as part of the multidisciplinary team within a sub-acute intermediate care setting to provide a seamless service to patients.
The post holder will continue to develop and undertake a range of delegated tasks and will report to a registered Nurse or Allied Health Professional.
The Reablement Worker will provide general / specific rehabilitation, reablement and personal care as specified below for a patient group and will be proficient and competent to work across professional disciplines. The reablement worker will also be supported to achieve competencies to enable them to work in-between the disciplines.
Successful candidates will be expected to work across a 24/7 shift pattern and the successful candidate will have the opportunity to learn, develop and practice a variety of skills, as well as being a valued member of our exceptional multi-disciplinary team.
