Waythrough is proud to be a provider within Reconnected to Health, delivering a drug and alcohol recovery service across all seven North East Prison establishments. Led by Spectrum Community Health CIC, Reconnected to Health is delivered by Spectrum, Tees, Esk and Wear Valleys NHS Trust and Humankind to provide holistic, collaborative care to men and women in custody across the region. We provide drug and alcohol recovery co-ordination, psychosocial interventions, and continuity of care planning within the integrated prison healthcare system. We work with community services throughout the country and have excellent links with all Northeast community drug and alcohol services. Reconnected to Health offers support to residents within a secure setting to recover from drug and/or alcohol dependence. The aim of the Specialist Healthcare Navigation (Enhanced Reconnect) service is to reduce health related barriers that directly impact on the individual's engagement prior to, and upon leaving prison; to reduce health inequalities and address the health and wellbeing of serious and high-risk offending behaviours. As an Enhanced Reconnect Navigator, you will promote continuity of care support for those leaving prison, encouraging positive and sustainable connections for service users within their local communities and building recovery capital assets. You will work with medium to high-risk offenders who will benefit from support to link in with partners and services and to promote access to a wide range of opportunities on release. You will also collaborate closely with key partners - including our Spectrum clinical partners - and colleagues across the street to support partnerships to achieve the best health and wellbeing outcomes for individuals. Recovery Co-ordination is a diverse and rewarding role, Key Duties include:

  • Manage an adult caseload of service users, delivering person centred care.
  • Co-produce and review recovery and treatment plans.
  • Facilitate and promote individuals' progression from initial engagement, through structured treatment, to self-sustained recovery.
  • Reduce alcohol/drug-related harm to individuals and the wider community.
  • Work with service users towards detoxes both community and in patient as well as rehabilitation centres.
  • Working collaboratively with key partners and colleagues to achieve the best health and well-being outcomes for individuals.
  • Internal candidates will still need to submit applications but can state they are internal applicants and ensure the applications hit the role specific criteria in terms of skills and experience. please note these roles involve regular evening work and occasional weekend work, 3 hours on a Saturday likely to be once every 3 months which is on a rota basis.
