Full Job Description

The Community Enhancing Recovery Team (CERT) was set up in 2014 to provide intensive, bespoke packages of care to service users in their own homes. Over the past 10 years the team has expanded to work with service users with mental health problems, personality disorders and/or learning disabilities to support them to move from hospital into their own tenancies. Referrals are primarily received from inpatient acute and rehabilitation services.

Service users are supported by a full multi-disciplinary team with the aim of enabling them to live independent and fulfilled lives. The team provides intensive quality interventions primarily between 8am and 8pm, 7 days a week to promote recovery.

Main duties of the job

The Community Enhancing Recovery Team (CERT) is a service that provides individual packages of intensive quality intervention to a diverse group of people who are or have experienced difficulties due to complex health and social needs, in their own home.

As a recovery worker you will be working intensively with a small number of people, enabling them to live their lives and achieve their own goals with support from a Multi-Disciplinary team.

People who use the service are from diverse backgrounds and will often have mental health needs, learning disabilities or personality disorders. They will often have had long or frequent hospital admissions.

All team members will have the opportunity to contribute to the development of the team and we will offer induction and training in a wide range of areas which will give you the skills to feel confident in your role.

You will have the desire and passion to improve the quality of life for people in all aspects of their lives which may have been disrupted by ill-health.

The aim of CERT is to prepare people for the next step in their lives where we hope that people will require decreasing levels of support.

CERT is a service that provides a 7 day a week service between the hours of 8am and 8pm, the post holder would be required to work flexibly within this.
