Full Job Description
We are seeking a dedicated and professional Referral Coordinator to join our Community Single Point of Access (SPOA) team on the Isle of Wight. In this role, you will conduct comprehensive patient assessments, provide expert advice, and direct patients to appropriate resources. You will serve as a key member of our inter-professional team, executing referral reviews and telephone triage, and collaborating with healthcare professionals to deliver high-quality services across a diverse range of community services.
As the initial point of contact for service users, you will provide a welcoming and supportive environment, guiding and supporting them throughout their treatment journey. You will advocate for service users, ensuring they are on the correct treatment pathway, and facilitate effective communication between staff and service users. This role requires a commitment to Health and Safety protocols, ongoing professional development, and may involve weekend and bank holiday work on a rotational basis. Engage in an integrated service model, delivering multifaceted interventions. Please note that this role may necessitate weekend and bank holiday work on a rotational basis in the future.
Uphold all Health and Safety protocols within the team to ensure a safe and secure working environment.
Contribute to the ongoing development of competencies and skills within the community setting.
Ensure regular participation in Continual Practice Development (CPD) activities to enhance the knowledge base of the referral coordinator.
Act as an advocate, adhering to the teams protocols, objectives, and procedures. Guide and support service users in navigating the correct patient pathway. Facilitate effective communication between the service user and all relevant aspects of community healthcare services. Advocate on behalf of the service user to establish a seamless connection between the service user and all facets of the Community healthcare services.
