Full Job Description

+ Be responsible for providing a single point of access for all referrals into Older Persons Mental Health Services, and be the lead specialist in this area.
+ Review all referrals to identify any gaps in the referral process, with the view to strengthen existing joint partnerships systems to improve early and appropriate referrals.
+ Collate and analyse the statistical data from the current database to facilitate the process to triage all referral information making autonomous clinical judgement in order to prioritise and allocate the referral to the appropriate professional within our service or suggest alternatives to the referrer. This is completed in line with the care pathway algorithm and risk management process and the Mental Health Measure.
+ Work as part of a multi-disciplinary team in supporting fellow colleagues within the community, to ensure the delivery of a high quality service for older people with mental health needs.
+ Work using own discretion within organisational policies and procedures.
+ May manage a caseload of patients in the Community.
+ Work collaboratively with local authority, third party sectors, and other disciplines within CTMUHB to provide and receive complex and sensitive patient related information and data.
In line with the All Wales Dementia Care Pathway of Standards this role will support the administration of collating statistics and the development of the dementia register., The successful candidate will work closely with the Older Persons Mental Health Service, and be the first point of contact for patients with potential mental health concerns. This includes but is not exhaustive to anxiety, depression, chronic enduring mental health diagnoses, stress related illness and cognitive decline.
You will also be responsible for carrying out initial contact and gathering relevant information in line with the UHB policies and procedures.
