Full Job Description

We have an exciting opportunity for a Regional Social Prescribing Lead to join our healthcare team across our Yorkshire. As a Social Prescribing Lead you will be responsible for the management of social prescribing co-ordinators within the region, as well as co-ordinating the social prescribing services across 9 Yorkshire prisons. Your main working location will be the prison based most locally to you, however you will be required to travel to all 9 sites as part of this role. Hours 9am-5pm Monday to Friday, As a Regional Social Prescribing Lead your responsibilities will vary. You will

  • Provide Leadership and management responsibilities for the social prescribing co-ordinators within the region.
  • Work in collaboration with wider regional team and site based service leads.
  • Provide robust reports of the activities undertaken and ensure the service is developed in line with report and audit outcomes.
  • Co-ordinate the social prescribing services across the Yorkshire prisons including the female estate.
  • Complete initial assessments with patients referred to and eligible for the social prescribing service.
  • Assessing their current wellbeing and identification of what matters to them.
  • Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing.

  • Experience offering an information, advice and guidance service in a health, care or similar setting
  • Experience of working within a prison environment or working closely with patient's with challenging and complex needs.
  • Experience of managing own caseload and understanding how to prioritise patient need and manage a complex caseload.

    This position is full time, you'll receive an annual salary up to £42,000 per annum - depending on experience. How will we support you?
