Full Job Description

We are looking to recruit 3 motivated and dynamic nurses with experience of working with older adults in primary or secondary care. The candidates must be dedicated to promoting the independence of patients/clients living at home/within the care sector to prevent hospital admissions. The post holder needs to be an expert generalist with highly developed knowledge whose role involves developing and maintaining a number of service interfaces aimed at ensuring individuals receive the right care at the right time and in the right place. The unit benefits from:

  • The department operates Mon-Fri 8am-4pm Cross cover across sites may be required. Informal visits encouraged.

    The ability to speak Welsh is desirable for this post: Welsh and/or English speakers are equally welcome to apply.,
  • work well within a multidisciplinary team providing a high level of oral and written communication skills.
  • Plan and deliver care to a high standard and evaluate effectiveness of care provided.
  • Use computer systems proficiently - Use of PARIS would be desirable.
  • Venepuncture and cannulation skills would be desirable.
  • Evidence of audit, research or service improvement.
