Full Job Description

We are seeking a registered nurse with a passion to make a difference for our older population.St David's Hospital specialises in rehabilitation and complex care planning. You will be part of an efficient and effective MDT, including volunteers and third sector who are all striving to provide a person centered care plan and assessment to aid rehabilitate and support discharge home.You will be a part of a dynamic team, promoting 'home first' approach.To work in our team, you'll need to demonstrate the core values as set out by Cardiff and Vale University Health Board; Kind and Caring, Respectful, Trust and Integrity and Personal Responsibility.You will be at the heart of everything we do, ensuring the patient receives the best possible care. In turn we will provide you with a friendly and supportive team as well as a full induction programme and an individual personal development plan to guide your career within your chosen role.We champion staff in academic learning, supported by
proactive educational teams throughout the UHB, enabling staff to maximise their potential in providing safe and effective care.Newly qualified staff will receive a supernumerary period to help you adapt to your new ward and role as a registered nurse., Deliver high standards of care to patients and be responsible for a patient for which you have been allocated. Care will be in keeping with the philosophies and operational policies of the Health Board. Act in such a manner to safeguard the interest of patients in your care. Follow the nursing process of assess, plan, implement and evaluate programme of care for patients considering their physical; psychological, social and spiritual needs. To escalate concerns about a patient's condition/deterioration to the most appropriate professional. Be aware of the legal requirement concerning the prescribing, control, security and administration of medication. Ability to delegate tasks to a range of staff. Awareness of Deprivation of Liberty safeguards, Mental Capacity Act and Mental Health Act and the ability to raise safeguarding concerns as appropriate. Ability to completed observations of patients in your care and ability to recognise deterioration.
