Full Job Description

The registered nurse will work within the Urology Clinical Nurse Specialist team and under the supervision of the senior CNS and will;-
+ Undertake nurse led follow up clinics for patients post treatment for prostate cancer, assessing patient's needs, implementing and evaluating care
+ Communicate any changes to care plan to the wider MDT and primary care
+ Support and manage the care of patients on self-management and remote follow up of prostate cancer
+ Act as the point of contact and named nurse for patients on self-management follow up., + Develop and provide patient education to ensure patients are fully informed about the prostate cancer self-management system/process.
+ Holistically assess patient's needs, developing and evaluation plans of care.
+ Mange the caseload of prostate cancer follow up patients, + Assess the patient's holistic needs and develop and implement a care plan based on patient need.
+ Communicate any changes or red flag symptoms to senior CNS and wider MDT as appropriate.
+ Provide advice and support signposting to other forms of support e.g. Welfare Rights, Community Teams, Maggie's
+ Provide advice and support to patients regarding side effects of prostate cancer treatments and ongoing symptoms and/or side effects
+ Liaise with the wider MDT and primary care regarding any changes to care
+ Liaise with primary care for the initiation on ongoing treatments such as hormone therapies
+ To be the trust advocate for self-management of prostate cancer follow up
+ Contribute to and participate in the ongoing service development of the self-management systems, the urology CNS team and the wider CNS teams.
+ Provide patient education about treatment related toxicities, symptoms to be aware of and how to access support.
+ Provide information to patients on self-management of prostate cancer follow up, address any concerns and ensure understanding
